Anyway so I study the pic more and I think its fake for three main reasons
1) The shit is on the front and not at the back
2) There are some stuff ON his pants at the front that u can see.
3) This looks to be more liquid that it has to be.
Dont ask me what it is cause I dont know. But I can quite confidently say its not him shitting his pants.
I also googeled it but couldnt find a legit news post about it.

in a new york marathon once, a participant did actually crap in his shorts but finished the race, so much human drama, how it transpired. this is 100% not fake fyi.
