It's pretty natural to desire someone who is fit, it means that they are healthy and you want to be with someone who can live a long relationship with you + take care of the family.

Does she have the potential to become better looking?

If yes, then you should let her know immediately that you want her to work out / become a better person for you to keep seeing her. You can say stuff like "it turns me on to see you working hard for me" and ask her to jog with you, you don't have to be a dick about it so long as your intentions (she has to become less fat) are clear from the start. If she is willing to do this, it can be a fair trade that she's working hard for you in return for your looking past her current weight and seeing the beautiful person inside.

If no, then you should probably friend zone her. Guys were built and designed to fuck healthy, beautiful women. She won't be sexually attractive for you, and sex is a major part of a guys' happiness. The relationship will never go anywhere if she keeps growing more fat.

As an aside, you should probably work on your game (and by game, I mean your value to women) if the best you can do is a fat woman. There are women out there that can give you anything you could ever wish for and you can have them. You don't need to be much of a guy to be better than most losers out there.
